Accommodations Letter Request Form

Students registered with the Learning Support Office should complete this form to request an Accommodations Letter. The Accommodations Letter outlines the specific adjustments or services for which you are eligible, and should be provided to instructors in person. Your letter will be available within five business days from the date of form submission.

Please note: It is your responsibility to request a letter each semester and to discuss accommodations with your instructor(s) in a timely manner.

Receiving your letter
Learning Support will email your Accommodations Letter to your student CCAD email account as a PDF attachment.
